Membership Terms & Conditions
Updated October 15, 2026
These terms explain current public plans and how they interact with your individual enrollment agreement. They do not retroactively reduce existing benefits or impose new financial obligations. Legacy founding tiers, credits and guarantees vary; the written terms actually accepted at enrollment govern. Confirm your plan's benefits and charges in writing before joining.

Discount Application Rules
Eligible for Founding Discount:
All Clinical Services at posted Boutique Standard Rates
Most retail Boutique products (exclusions may apply)
Treatment packages when purchased at regular pricing
Not Eligible for Founding Discount Stacking:
Flash Sales or limited-time promotional pricing
Third-party vendor promotions (e.g., manufacturer rebates)
Gift certificates or value cards purchased at a discount
Services already discounted through insurance, employer wellness programs, or other external sources
When both a Founding Member discount and a promotional price are available, you may choose whichever provides the greater savings. However, discounts cannot be combined or stacked unless explicitly stated in the promotion terms.
Enrollment and recurring payments
Before enrollment we disclose the amount and billing frequency, included benefits, any initial charge, minimum commitment, renewal term, cancellation procedure and any early-termination fee. Recurring charges require affirmative authorization; viewing this page, receiving care or providing a card alone is not enrollment. We provide a retainable acknowledgment of the accepted terms and cancellation instructions. Monthly charges ordinarily recur on the enrollment date or the corresponding billing date disclosed for a short month; annual plans recur only as authorized. Required renewal and material-change notices will be provided.
Cancellation
You may submit a cancellation request through an available Member Portal cancellation function, by email to contact@aevua.com, or by mail to Aevua Medical Spa, Attention Membership Services, 449 W Mt Pleasant Ave, Suite 1, Livingston, NJ 07039. Call (973) 313-5601 for assistance. We record the request date, confirm the effective date and explain any remaining authorized amount. Do not send full card numbers by email. An unavailable portal does not prevent cancellation by another accepted method.
Our ordinary enrollment terms call for 30 days' advance notice, subject to the agreement you actually accepted and any applicable statutory right to an earlier cancellation. We do not restart or extend the notice period because of an administrative delay. Billing stops at the effective cancellation date; prepaid benefits ordinarily continue through the paid period unless a lawful refund or other arrangement applies.
Minimum commitments and fees
A minimum term or early-termination fee applies only if clearly disclosed and accepted at enrollment and legally enforceable. Published plan descriptions have included $100 for Glow or Reserve during an applicable minimum term and $300 for Diamond or certain founding agreements. Legacy founding tiers may differ; this page does not replace their actual agreement or increase a lower agreed fee. No such fee applies after the accepted minimum term ends. We explain the basis before charging. We do not impose a new retroactive discount recapture, combine inconsistent penalties, or invent a minimum term from this website alone. Statutory cancellation, disability, death, inability to provide services and other applicable rights remain protected.
Credits, rollover and gift value
Eligible service credits accrue according to the plan's paid billing cycle. Standard rollover continues while the membership is active. Promotional bonus credits, paid service value, prepaid packages and purchased gift cards must be separately identified because their terms and legal protections differ. Any post-cancellation restriction applies only to the extent clearly agreed and lawful; membership termination is not an automatic forfeiture of protected paid or gift-card funds. Purchased gift-card funds retain applicable New Jersey and federal protections, including rules on expiration, fees and any required cash redemption. We do not describe a 24-month expiration as compliant with gift-card law.
Buddy or family passes are governed by the actual tier, permitted quantity, discount and exclusions. No general gifting paragraph expands or cancels a tier-specific promise. Recipients need their own assessment and consent. Credits and guest benefits do not guarantee suitability, treatment on demand or availability of a particular clinician. No service is provided when clinically inappropriate.
Payment problems and suspension
Keep payment details current and contact us about a failed payment. We may notify you, pause benefits and discuss payment or cancellation as permitted by the accepted agreement and law. A failed-payment or reactivation fee must have been disclosed, agreed and lawfully chargeable; this page does not authorize repeated $25 attempts or a new $50 charge without that basis. We do not impose an undisclosed fee merely because an automated retry failed. Access to clinical records and necessary follow-up is handled separately from membership billing.
Changes and existing benefits
Standard plan changes are communicated at least 30 days in advance where that is the agreed notice period, or earlier when law requires. You may cancel without a new penalty in response to a price increase as provided by the applicable agreement and law. Posting alone does not authorize a higher recurring payment where additional assent is required. Existing founding discount, price-lock and credit promises remain governed by the actual enrollment documents; no website revision silently revokes them. Advertising or sales errors discovered after payment receive an appropriate remedy.
Appointments, refunds and closure
The Appointment & Booking Policy describes the standard $40 deposit and 48-hour/50% cancellation rules. Existing written member grace-cancellation or credit-preservation exceptions apply; we confirm the applicable exception before charging. We will not apply conflicting general website descriptions to enlarge a fee. Refunds follow the Refund & Cancellation Policy, the enrollment agreement and law. Ordinary voluntary mid-cycle cancellation is not prorated unless the agreement or law provides otherwise. If we cannot supply prepaid services, we offer a client-accepted suitable alternative or the required unused-value refund. A practice closure does not automatically reduce unused paid value by half.
Conduct and termination
We may address threats, abuse, unsafe conduct, material nonpayment or other legitimate breaches through appropriate restrictions or termination. Any loss of future membership benefits is subject to the agreement and law. Ending membership does not extinguish refund rights, records access, necessary follow-up or professional duties concerning continuity of care. Good-faith complaints, chargebacks, disability accommodation requests and protected privacy choices are not grounds for unlawful retaliation.
Clinical independence and annual forms
Membership is a payment and benefits arrangement, not consent to treatment or a guarantee of outcomes. Prescription treatments require individual assessment and consent. Patients ordinarily renew forms every 12 months; staff confirm changes and the current plan at each visit and obtain updated specific consent when needed. Neuromodulator units are product-specific and not interchangeable. Confirm the product, dose, total price and applicable benefit before treatment.
